How you become a patient

Your doctor, urgent care, or specialist faxes us a referral at 203‑675‑0277. Then our team calls you to schedule. Haven't heard from us? Call 203‑675‑1199 — you will not be left waiting in silence.

What to bring

Photo ID, insurance card, a list of your medications (or the bottles), and any imaging or records you have. Bring a working email address — it connects you to your patient portal.

Running late? Here's our policy

Call us as soon as you know. We'll tell you honestly whether we can still see you or need to reschedule — no wasted trips.

Why we ask everyone for a lab test

It's the same safety step for every single patient — it helps us confirm your treatment is working the way we expect and keeps your care safe. It is never a judgment about you.

Medication questions

When medication is part of your treatment plan, your pharmacy can answer most day-to-day questions faster than we can — start there first.

For anything your pharmacy can't resolve, send a message through your patient portal and we'll respond during business hours — no hold music.
